Celebrate small successes.

Even with the smallest goals being accomplished, celebrate them. Give yourself a pat on the back when something good happens. Tell your loved ones about how things are going. Reward yourself. It's good to keep moving forward, but you have to stay sane in the process.
Start small with your goals, accomplish them, celebrate them, and then set bigger goals. Once the motivation starts, it'll only get greater and greater with each accomplished goal.
